Telehealth Informed Consent

Telehealth means receiving healthcare services remotely, using electronic communications instead of an in-person visit. Before receiving care through providers connected via Feel20, you should understand the following.

What telehealth involves

Your provider will evaluate you using the information you submit and live or asynchronous communication (video, phone, or secure messaging, depending on your state's requirements). They may prescribe treatment, recommend in-person care, order labs, or determine that treatment is not appropriate for you.

Benefits and limitations

Telehealth improves access and convenience, but it has limits: your provider cannot perform a hands-on physical exam, and technology can fail or be interrupted. In some situations an in-person visit is clinically necessary, and your provider will tell you if that's the case.

Your responsibilities

Telehealth is only as safe as the information you provide. You agree to give complete, accurate health information — including current medications, allergies, and medical history — and to update your provider when things change.

Your rights

You may withhold or withdraw consent to telehealth at any time without affecting your right to future care. You may ask questions about any aspect of your treatment, and your provider must answer them. Your records are protected as described in our Privacy Policy and under applicable law.

Emergencies

Telehealth is not for emergencies. If you experience a medical emergency, call 911 immediately.

By submitting an eligibility check and proceeding to a consultation, you acknowledge that you have read and understood this consent and agree to receive care via telehealth where permitted by your state's law.
